Late Withdrawal Request
 
Policy on Late Withdrawal Request: Students may request academic concession in circumstances that may adversely affect their attendance or performance in a course or program (e.g., significant medical issues or life events). A late withdrawal (W standing on transcript) is typically granted after the withdrawal deadline has passed and the student has not met course requirements and is unable to do so due to extenuating circumstances. Students wishing to request academic concession must apply as close as possible to the time attendance is adversely affected. Complete this form only if the date to withdraw on your own has passed. Complete all relevant sections and attach supporting documentation (e.g., medical or extenuating issues documentation).
