Withdrawal Request Form - UBC Applied Science Co-op Program

Completion of this form is required to withdraw from the co-op program. 

Requests to withdraw from the co-op program must be received prior to accepting the Terms & Conditions on PD Portal for each job search term and by the following dates: 
  • For a Winter work term (January to April) by August 31
  • For a Summer work term (May to August) by December 31
  • For a Fall work term (September to December) by April 30

Note:  As per the Terms & Conditions, you are not permitted to withdraw from the program after: 
  • you have accepted an employment offer, for a position secured through the Co-op Program or through your own endeavours
  • while on a work term
  • and/or after employment has been confirmed for a subsequent work term with the same employer
Enrolment in the co-op course(s) will be maintained and you are responsible for all assessed fees for the duration of the employment contract.

International Students Only

The co-op work permit can only be used for work that is a required part of your academic program such as an official UBC co-op work term that is recorded on your academic transcript. By withdrawing from the co-op program, you understand that the co-op work permit cannot be used for work that is not part of your official academic program of study for any employer.

By submitting this form, you confirm that you understand the process to withdraw from the Applied Science Co-op Program and you authorize the Applied Science Co-op Program to remove the co-op option from your academic record.
