Field Education Application Form - Part B

It is important for students to note that the use of this "Field Education Application Form - Part B" is for the Field Education Office to review and use the information provided to facilitate the best fit between the student learning goals and agency criteria. This form will not be shared with outside agencies; however, content from it might be summarized and provided. 

Late Field Education Application Forms are highly discouraged and the action is considered unprofessional. If a student feels that they have a strong rationale for a late submission, they must submit an Exception to Field Policy Request Form at the same time. The Field Education Office begins the matching process for Winter Term in August and for Summer term in late November/December, and thus students who submit a late form may not have their interests, previous experience, and learning goals considered. 

If you require exceptions to Field Education Policies and Procedures outlined in the Field Education Handbook, please submit an Exception to Field Policy Request Form. 

If you require accommodations for Field Education, please speak to the Field Education Coordinator and also contact the Disability Resource Centre to initiate the intake process. Note that students should be descriptive and informative, yet concise in their responses for this form. To ensure students are informed when indicating agency preference, students are encouraged to review the Community Partners List and their websites prior to completing the form.
